Directions to Kirkland/Totem Lake Location
From I-405 Southbound
» Take the NE 124th Street Exit (20B). At the end of the ramp, turn left onto NE 124th St. Proceed east, crossing over the freeway overpass. Continue a few blocks to Totem Lake Blvd.
» Turn left onto Totem Lake Blvd.
» Get in the right-hand lane and proceed one block to 120th Ave NE.
» Turn right onto 120th Ave NE.
» Continue north on 120th Ave NE (up the hill, passing the hospital) until you reach NE 132nd St .
» Turn right onto NE 132nd Street and immediatly take the first driveway on your right.
» Evergreen Place building is on the left once thorugh the driveway. Futura Health Screening Lab is on the ground level and located toward the center of the first set of buildings.
From I-405 Northbound
» Take the NE 124th Street Exit (Exit 20B).
» Stay to the right on the exit ramp, taking the “Totem Lake Blvd” ramp (follow blue hospital signs). Go straight through the stoplight at Totem Lake Blvd and onto what has become 120th Ave NE.
» Continue North on 120th Ave NE (up the hill, passing the hospital) until you reach NE 132nd St.
» Turn right onto NE 132nd Street and immediatly take the first driveway on your right.
» Evergreen Place building is on the left once thorugh the driveway. Futura Health Screening Lab is on the ground level and located toward the center of the first set of buildings.
Directions to West Seattle Location
» From I-5 North or South, take the exit for the West Seattle Bridge (Exit 163).
» Continue West along the West Seattle Freeway and over the bridge.
» Continue onto Fauntleroy Way SW.
» Take a right on SW Alaska St then a left on 42nd Ave SW.
» Once in the Jefferson Square building lobby, take the elevator to the 4th floor. Go right from elevator on 4th floor, suite 488.
Directions to Yakima West Location
» From Hwy 12 take the 40th Avenue exit.
» Continue on 40th to Englewood (top of hill).
» Turn left on Englewood Ave.
» Turn left at 39th Ave.
» Turn left at the “T”.
» Turn left into the parking lot.
